Initial Appointment


Individuals come to the Human Performance Center in a variety of ways. You or a family member can contact us and set up an appointment. Or your physician or can refer you by giving you a referral or faxing one to us.

Your Initial Appointment


Call us to set up your appointment at 805-687-8553. We will request some general information, including insurance information. We will use this information to verify your insurance benefits. Simply click on the Insurance Information submenu on the left for more information about insurance, or call or email our office manager, Rachel Mari at 805-687-8553x 13 or rhuhn@hpcsb.com


Please arrive 10 minutes early for your first appointment to fill out paper work. We will copy your insurance card and you will be asked to complete a survey regarding your symptoms and how they affect your function. This will better allow our therapist to individualize your treatment program.


You may also download the paperwork and complete it at home prior to your visit. Simply click on the New Patient Forms submenu to your right and download the forms pertinent to your specific type of insurance or condition.


What to expect when you meet with your therapist for the first time


Physical Therapy is a form of health care that improves movement and function, relieves pain, and enhances health and wellness through physical interventions.


Care by your physical therapist at the Human Performance Center involves

  • A thorough examination.
  • Formulation of a plan of care.
  • Interventions – therapeutic or preventive.
  • Individual instruction & education
  • Home Program/Community Exercise

Based on your initial evaluation and your desires, other health care professionals may be included in your plan of care such as an occupational or speech therapist, dietitian, marriage and family or social service counselor.


Your therapist will encourage you to become an active partner in your care as opposed to a passive recipient. This may include performing home exercises or, improving your posture, using appropriate lifting mechanics, or modifying your work environment.
The number of visits you receive will depend upon your physicians request, your response to treatment, and your therapists’ assessment of your need for ongoing treatment.

HPC Offers Several Payment Options


As a HPC patient, you can expect the HPC to go the “extra mile” when it comes to insurance coverage and billing issues. As an exclusive preferred provider for the national PTPN network, the HPC accepts most major health plans and dozens of others (see below). The HPC continues to work with insurance companies to determine benefits and handles all billing for the patient.


We bill Medicare primary and secondary insurance, private insurance, PPO’s, HMO’s, workers compensation and personal injury. We also accept personal payment plans by check or credit card.


We do not require a physician referral to be evaluated at HPC. However, to bill your insurance we require a medical diagnosis, usually from a physician.

Information About Our Facility


The Human Performance Center(HPC) is a Medicare Certified C.O.R.F. (Comprehensive Outpatient Rehabilitation Facility) and is located at Santa Barbara California.


Our license allows us to provide and bill insurances for Physical, Occupational, and Speech Therapy, Social Services, Psychological Counseling, and Respiratory Therapy Services. We also work with registered dietitians for nutritional counseling. Sharon Basham, MD, is our medical director and meets with the staff monthly during a patient care conference.


Our staff includes 8 physical therapists and 1 physical therapy assistant as well as many other health care professionals. Feel free to “Meet our Staff” elsewhere on our website. Over 200 different physicians from multiple specialties referred patients to our facility last year. 90% of our referrals are from physicians and 10% self-referred.


Our Mission is to provide dynamic health care services that improve movement and function, relieve pain, and enhance health and wellness for those of the greater Santa Barbara area. We help our clients regain and maintain an active lifestyle.


In June, 2004 HPC moved to our current location at the corner of Calle Real and Pueblo, just two blocks from Cottage Hospital in the heart of Santa Barbara’s “medical district.” Our beautiful, expansive 5,500 square foot facility was designed from day one with patient comfort and privacy in mind. It features a relaxing, airy patient waiting room, 10 private treatment areas and the best exercise and rehabilitation equipment available today. The high ceilings and frosted-glass windows of our main treatment gym offer a very spacious, pleasant environment for exercise and rehabilitation.
